Patient Portal

In order to use our Patient Portal, you must first be assigned a user name and password. Please call us to obtain your user name and password.

Once you have been assigned your user name and password you can use the Patient Portal to do the following:

Send and receive messages
Request a new appointment and view upcoming appointments
Request a prescription refill from a list of currently refillable prescriptions
Examine current and past statements
Enter or modify personal information and other demographic information
Receive reminders from us about when the next lab/visit is due
List of prior referrals and ability to request new or existing referrals
Receive email: for reminders, upcoming appointments, and statements

Please change your password the first time you log in.

For urgent matters after hours, you can reach the answering service to contact the on-call provider by calling the main number at 972-981-7500. Your call will be forwarded to the answering service Monday through Thursday starting at 5:00 PM until 7:30 AM the following morning, as well as Friday starting at noon through Monday at 7:30 AM. Non-urgent matters will be handled through the patient portal 7 days a week or during regular office hours via telephone. If you have a true medical emergency, please go to the nearest ER or call 911.

Messages left for the nurse will only be checked Monday through Friday, during business hours, before 3:30 PM.

Please allow 24 business hours for a response to your message. Due to the requirements of federal and state laws governing the privacy of patient health care information, your message will not be returned via email.
